The record

General Owoye Andrew Azazi (1952–2012) was the 13th Chief of Defence Staff of Nigeria (2007–2008) and the National Security Adviser from 2010 to 2012. He died on 15 December 2012 in a Nigerian Navy helicopter crash near Okoroba in Bayelsa State together with former Kaduna State Governor Patrick Yakowa.
